One-year follow-up of the RESiLIENT trial of smartphone CBT is out (n=3,280): CBT skills reduced major depression onset risk by 37–48%. But depression waxes and wanes. It appears that regulatory agencies are pretty ignorant of predictive/diagnostic modeling. A very similar thing happened with the Japanese PMDA.

This was a most illogical contrast one could ask. A clinician treats Mr A, and is interested in how they change over time. So a within-person change. But in deciding how to treat Mr A, the clinician must contrast Mr A receiving treatment X and not receiving it. Because we cannot do this (continue)

Agree. However, they found one significant interaction treatment x sex: a larger drug-placebo difference between men and women on day 2. HTE? Ore more likely a false positive finding, given there were 4 subscales of the MADRS and 6 time points and no adjustment of alpha pubpeer.com/publications...

I think they are even worse than that. They showed that sex is a prognostic factor. But they don't understand that prognostic factors essentially have no place in precision psychiatry. Precision psychiatry is about treatment*covariate interactions, i.e. about effect modifiers or HTE.

Precision psychiatry? Study on gender-specific esketamine response. Some gender diffs but irrespective of group (esk/placebo). Only one significant gender x treatment interaction, on a specific day, with a specific outcome: p = 0.02. It did not occur to authors that this is likely a false positive

Today we published a survey of the SWD (smallest worthwhile difference) of statins, which found that the median SWDs exceeded its actual efficacy across all risk scenarios, suggesting that laypeople demand greater benefits than statins typically provide. jamanetwork.com/journals/jam...
