🥺 And now, the time has come...it's nearly the end of #BIGSPD26 @bigspd.bsky.social Co-Presidents Fi Kuhn-Thompson and Gary Lamph are giving a few reflections.

A British and Irish network of academics/clinicians/lived experience/carers interested in the field of ‘personality disorder’. Follow #BIGSPD26 for our live reporting throughout conference!
It’s our final day and the last keynote talk of our conference is underway. Peter Coventry talks us through his research exploring the importance of nature based inventions building social connectedness, skills acquisition & health benefits including increased vitality and strength #BIGSPD26
Our closing keynote is from Professor Alison Leibling considering and conceptualising the ‘moral’ environment within prisons and using psychologically informed planned environments. #BIGSPD26
Today at #BigSpd26 we're running a thought experiment: if you had £1 million to improve outcomes for people experiencing mental distress, how would you spend it? Click here app.sli.do/event/xgQMZa... or scan the QR code to join the conversation or share your ideas in the comments below.
Dr Mary Ryan and Dr Rachel Gibbons are our next keynote, helping us understand the patient/clinician role and whether it’s a helpful illusion or harmful delusion. Exploring how we might feel in our roles and how label change us alongside their work with the prejudice and stigma project #BIGSPD26
We were indeed. All the way back in 2014! #BIGSPD26
We co-chaired (as a clinician & patient) a RCPsych Working Group, and for years we were the only pair with those roles doing this within the College. This has now changed. I think @bigspd.bsky.social were also the first national body to have Co-Presidents with lived & clinical experience #BIGSPD26
A quick opening from our co-presidents and now we’re into Raf Hamaizia’s keynote talk about lived experience & co-production: a bridge of mutual understanding. Raf kicks off his talk with sharing a video sharing parts of his journey through life, services and into lived experience working #BIGSPD26
Head down to the CHT stall in our exhibition hall while grabbing lunch or a coffee and share your thoughts and experiences of the label. Always a rich discussion point! #BIGSPD26
On day 1 of #BigSpd26 & on our stand we're asking for people's views on the term "personality disorder." Some find the term validating or useful, while others feel it misses the complexity of their experience. What do you think? Share your thoughts below & we'll share full results later today.

Here's the orthodoxy according to Ad de Jongh: NICE says psychological treatment for personality disorders should last at least a year. Yet almost half of patients don't respond well and about a third drop out. What if shorter, targeted therapy could work? #BIGSPD26
Questions section about silos from audience for first speaker, who works with the Blackpool NHS service. She asked why the service isn’t well known outside its remit, and if it will be grown to link to other services later. #BIGSPD26 Hard to to do with NHS budgets!
There's little evidence that DBT, MBT or Schema Therapy beat treatment as usual, and no evidence that longer treatments beat shorter ones for personality disorders, says Ad de Jongh #BIGSPD26
Why focus on trauma? Childhood adversity isn't just a risk factor for personality disorders, says de Jongh, it may be a central driver. In one study each ACE raised borderline PD risk fivefold. Emotional abuse and neglect were the strongest predictors. #BIGSPD26
An intensive 8-day trauma-focused programme (prolonged exposure + EMDR) for people with PTSD and borderline PD. One year on, 73% no longer met criteria for borderline PD. Dropout just 1%. We couldn't believe our research results, says Ad de Jongh #BIGSPD26
We need to focus on Adverse Childhood Experiences, says de Jongh. Using Shapiro's Adaptive Information Processing. A negative self-image like 'I am worthless' isn't the cause of dysfunction but the result of unprocessed memories. Treat the memories, shift the symptoms. #BIGSPD26
Childhood adversity is not just a risk factor for 'personality disorder', but it is a central driver, argues de Jongh. #BIGSPD26
de Jongh explains how adverse childhood experiences can lead to latent vulnerability, which in turn lead to social thinning #BIGSPD26
The TEMPO study looked at EMDR therapy for people with personality disorders, with and without PTSD (N=159). Remission of PD diagnosis rose from 44% to 65%, with a 5% dropout. #BIGSPD26 pmc.ncbi.nlm.nih.gov/articles/PMC...

Headline figure from Ad de Jongh's 6-day PSYTREC programme: 78% of people no longer met criteria for their primary personality disorder at 12-month follow-up. Improvements held across all PD clusters. #BIGSPD26
To identify memories linked to specific symptoms, the following questions may be helpful, says de Jongh #BIGSPD26
This is Ad de Jongh's big question for #BIGSPD26 Can short-term trauma-focused treatment help people with a personality disorder change? His answer: yes. Brief, intensive care delivering large and sustained gains, and a challenge to how we commission 'personality disorder' services.
