I had the honor of meeting Jeffrey Cooper @ the last ASA meeting & got my hands on the chapter from the 1974 NATO conference “The Anesthesia Machine - an Accident Waiting to Happen” 50yrs later it's still applicable - The need to embed human factors experts in patient safety has never been stronger.
