Jia Arng Lee presented a puzzling case: a 40-year-old Taiwanese man with fever, cough, fatigue, and anaemia after a trip to Japan. Despite testing negative for major infections, his condition worsened by day 18. Bone marrow, CT, and labs pointed to HLH - prompting expert debate.
