Every style covered so far — APA, MLA, Chicago, AMA — puts the surname first. IEEE flips it entirely: J. M. Smith, not Smith, J. M. It's the single most counterintuitive rule across all five styles, and the easiest one to get backwards without ever noticing you did.

AMA and Vancouver are close enough to get confused constantly — same numbered superscripts, same NLM abbreviations. One easy-to-miss difference: AMA truncates author lists to the first 3 once you hit 7+. Vancouver keeps going to 6. Same rule, different number, quietly wrong list.

A citation doesn't have to be fake to be wrong. A real finding gets cited by a second paper, slightly overstated. A third paper cites the second paper instead of the original, and inherits the overstatement as fact. Nobody invented anything — the claim just drifted.

Chicago style requires the city of publication for every book — and some of the "obvious" ones aren't obvious at all. Yale University Press is in New Haven. Not New York. A checker that only verifies formatting wouldn't catch a city that's simply wrong.

Read your reference list bottom to top. Read your manuscript end to beginning. Sounds like a party trick, but it works: your brain auto-corrects familiar text when reading normally — exactly why the same typo survives ten read-throughs.

APA puts the year right after the author's name. MLA skips the year in-text entirely and gives you a page number instead. Neither is arbitrary: APA assumes you need to judge if a finding is recent. MLA assumes you're pointing someone to an exact passage.

A desk rejection doesn't mean reviewers weren't convinced by your paper. It means no one with subject expertise read it at all. An editorial assistant checked your reference list against the journal's technical requirements, and that's as far as it got.
