The earliest citation for the phrase would seem to be Chaucer's *Troilus and Cressida*, "Lat nat this wrechched wo thyn herte gnawe, But manly set the world on sexe and seuene” which Shakespeare had read. At first in meant "on a throw of the dice". By Sh.'s time it meant a state of confusion.

It's easy to dismiss young people for being unread couths, but when they say "six-seven" they are in fact referencing lesser known Shakespearean drama (Richard II, act 2, scene 2).
