This is a conversation our specialty needs to have. The indications for AFOI have massively decreased as proficiency with VL has increased. Even when performed well AFOI is less pleasant than asleep intubation so hard to justify performing to train or maintain skills.

Hang on a minute. The way I read this, the letter's authors' seem to be claiming that because the emission of desflurane is decreasing that therefore it's OK to keep emitting more desflurane. The climate cost of emitting more desflurane today is unrelated to previous emissions.
