For the last few decades, through agile and lean schools of thought, systems theory has been a dominant model in software dev thinking - how do we optimise for "flow" I now think a more useful model lies in information theory - how do we optimise for *learning*? Not really the same things.

Broken incentives. A broken, corrupted business environment. A broken dominant cultural paradigm. And frankly, a broken culture.
Quite rightly, UK businesses are being held to higher standards of traceability for interested parties. Maybe time that happened in politics, too?
