We're excited to share a new research publication from Dr. Jon Wilson, @glombandit.bsky.social, Dr. Randy Haun, and Dr. Chris Larsen titled "Framework for Apolipoprotein 1-Mediated Kidney Disease Classification." Read here: https://www.kireports.org/article/S2468-0249(26)02639-2/fulltext #nephsky
Framework for Apolipoprotein 1-Mediated Kidney Disease Classification
Black Americans have an increased risk of chronic kidney disease (CKD) compared with individuals without African ancestry. Variants designated G1 and G2 in the apolipoprotein L1 protein encoded by the APOL1 gene are a major contributor to this increased risk.1 These variants arose independently, rar...

We're proud to share a new publication with @aaronstorey.bsky.social, Dr. Patrick Walker, Dr. Chris Larsen, and @glombandit.bsky.social, titled "Serine Protease HTRA1 is a Common Autoantigen in Membranous Nephropathy in Patients of 80 Years and Older."
Serine Protease HTRA1 is a Common Autoantigen in Membranous Nephropathy in Patients of 80 Years and Older - PubMed
HTRA1 is a common autoantigen in PLA2R-negative MN among very elderly patients, occurring in approximately one in five cases aged 80 years or more. These findings support inclusion of HTRA1 testing in diagnostic evaluation of antigen-negative MN in patients 80 years and older and suggest the existen …

Proteomic Profiling of Complement Components in Glomerular Disease - PubMed
The complement system plays a central role in glomerular disease development and resolution. Currently, renal biopsies assess the presence of complement through limited immunofluorescence stains (C3 and C1q). With more than 50 total proteins and fragments involved in the complement cascade, this met …

@nephrosharma.bsky.social and @glombandit.bsky.social sat down with Dr. Salil Parikh, a radiologist who helped his facility improve their adequacy rates significantly, to discuss one possible blueprint for better renal biopsies. #nephsky #radiology Listen to the conversation here:

IgA Vasculitis A kidney biopsy is mandatory in patients with persistent proteinuria ≥0.5 g/day for more than 4 weeks, impaired kidney function, or suspicion of rapidly progressive glomerulonephritis (RPGN). #NephJC
The evidence of immunomodulatory effects of steroids and Nefecon comes from the following trials. High dose steroids treatment suffered from complications like infections. Nefecon, however, still showed similar systemic side-effects of other oral steroids. #NephJC
Treatment Approach: 💊 Novel therapies for IgAN have shifted the paradigm to a bi-pronged approach. a) Generic responses to HTN/proteinuria b) Fundamental drivers of nephron loss by reducing the formation of aberrant IgA molecules & the maladaptive mechanisms that contribute to CKD. #NephJC
Global Implementation of IgAN 2025 guidelines: #NephJC As we learn more and develop new treatments, it is important to try to make these available to a wide population of patients. Including low-income patients in future studies might allow for at least some access to cutting edge therapies.
First things first — where the story starts! Time to unpack the 4-hit hypothesis of IgA nephropathy — understanding each step opens doors to new treatments. #NephJC

T3j 🧘 eGFRdiff is weakly/not affected by 🚫 Fat free lean muscle mass 🚫 Inflammation 🚫 Shrunken pore syndrome 🚫 Obesity 💡 This lack of alteration due to systemic issues makes eGFRdiff attractive for use in a wide variety of patients. #NephJC
Inflammation—> increase CRP —-> CRP affects Cystatin C —> reduced Cystatin C eGFR —> negative eGFRdiff #NephJC
T2f Let's deep into the prospective analysis 🥽 For each 10 mL/min ↑ in eGFRdiff 🚨20% ↓ mortality risk (AHR 0.80 [95% CI: 0.74–0.88]) 🚨17% ↓ HF risk (AHR 0.83 [95% CI: 0.73–0.94]) ✅ Association held after adjustment for muscle mass & filtration markers. What do we think? #NephJC
T0f So why does this matter? A more negative eGFRdiff (eGFRcr > eGFRcys) is linked to: 🫀 Cardiovascular disease 🧬 Frailty 💓 Heart failure ⚰️ Mortality #NephJC
T0e But muscle mass isn’t the whole story. 🧨 Factors contributing to eGFRdiff include: Chronic inflammation Endothelial dysfunction Shrunken pore syndrome Obesity Smoking Corticosteroids Which others do you consider? #NephJC
