Doxycycline postexposure prophylaxis for bacterial STIs: Prescribing patterns, use, short-term outcomes among 2083 patients in a Los Angeles Federally Qualified Health Care Program, and implications for widespread use.

Rates of bacterial sexually transmitted infections (STIs) have risen dramatically over the past decades. Doxycycline postexposure prophylaxis (DP) is a novel intervention to prevent bacterial STIs. Recent randomized controlled clinical trials reported high DP efficacy at preventing syphilis and chlamydia in cisgender men who have sex with men (MSM) and transgender women.

as someone about to graduate ID fellowship, the NIH price freeze immediately prompted my “how does this affect my k23 application?” But today I realized I’m fortunate to always have clinical medicine to do, as opposed to many community workers and research workers fully dependent on NIH financing.

I had a lot of fun riding this one with @anuhazramd.bsky.social , definitely a topic with some controversy, but could be a pivotal move to help reduce the incidence of this (not so) “hidden” syphilis epidemic.

Many infants and children who are infected with hepatitis B become chronic carriers. Since we have been vaccinating newborns since the 1990s, the number of new cases of hepatitis have decreased. (3/7)

The ID march of shorter treatment is ok continues, this time for bacteremia (not including S. aureus). FYI, E. coli, Klebsiella, Enterococcus, and CoNS made up > 70% of organisms. By pathogen breakdown of risk difference emphasizes applicability of these results for GN bacteremia.

BALANCE is out! 7 days of antibiotics non-inferior to 14 days for bacteremia (excluding S. aureus). this is a practice-changing study that should have huge impacts on antibiotic prescribing. i also can't say enough about the study team (esp. nick daneman). inspiring. www.nejm.org/doi/full/10....
