Had my first ever meeting with NASA human spaceflight operations today, and it was very interesting. The guy opened with an extensive section that essentially said "Here are photos of 17 dead astronauts. All of them could still be alive today if people in operations had done something different."

Dealing with a piece of undocumented software where IP addresses are written like "192.168.0.1.68". You might reasonably assume that that 68 is a TCP/UDP port (as I once did), but you would be wrong!
