Esme'kath is essentially the outcome of Bhaal's "fuck it, we ball" moment. Githyanki are a warrier race. Barbarians are...well...barbaric. Put the two together, sprinkle in some cannibalism, remove empathy and any sense of composure, and you get the ultimate killing machine, unhinged and unyielding.

Andromeda was created the canon way...sort of. She is the end product of centuries of planning. Bhaal started with dragons formed from his essence, then those dragons mated with humans and elves formed from his essence. The lineage went on until he had the perfect specimen (1)
