There’s a story out in the last few days about someone on clozapine who died of clozapine toxicity because briefly stopping smoking increased his body’s absorption of it. It’s a horrible story and highlights some really huge gaps and conflicts in psychiatric practice - eg there is a HUGE push to get

This does seem to be a key barrier when people ask how can we get the best possible people in parliament. Being a candidate and then a backbench MP looks pretty awful to me. "Why We Get the Wrong Politicians" by Isabel Hardman really opened my eyes to this and changed how I think about politics.

The visible, and sometimes openly expressed attitude of 'these people should be grateful. Last week they were a teacher and now they are an MP with a precarious majority and who gets death threats and called a paedo protector'. They liked being a teacher!

There are *so* many examples where the onus is on vulnerable patients to navigate poorly functioning systems. There are sometimes relatively simple fixes that could make things better if staff had time to pause and consider their service as a whole. But that's rare, and everyone loses as a result.

Not only is he managing a chronic condition, he has this additional admin burden. It causes unnecessary stress & makes the condition feel far more onerous to live with. There could be tons of examples like this where navigating the system is almost as stressful as having the condition itself 2/2

Prohibition has never worked. We already know that very high numbers of under 16s in Australia continue to access social media notwithstanding the 'ban'. The same will happen in the UK. All a ban does is given the impression of having 'done something' while ignoring the actual issues.

Don't assume that people know they're appreciated. Seems to me genuine appreciation & being seen for making an effort at something really does land. It's something we can do for each other, & can make more difference than we know.

There is understandable concern with potential risks of data sharing in the NHS. There is also actual & real harm that happens regularly because there isn’t enough NHS data sharing. Changes to improve data sharing for NHS & social care services will make real positive difference to people’s lives.

Minister Zubir Ahmed, a transplant surgeon, told me that in January he had to decline a kidney for someone because a lack of access to GP records at a weekend meant he couldn't be sure the organ was safe. GPs meanwhile still rely on letters from hospitals to know what happened to their patient.

Beautiful piece of writing about the importance of things we can't easily measure. Continuity of care increases the chances that people are better understood and cared for: "The whole family, held in the same building by the same person, long enough for the shape of their lives to become visible."

Tackling polypharmacy in primary care: Reflecting on learning from the IMPPP clinical trial webinar

Polypharmacy (taking five or more medications) is a major challenge facing primary care today, with implications for patient safety, treatment burden, and NHS resources. Yet there is limited high-quality evidence to guide how best to manage it in everyday practice. The Improving Medicines use in People with Polypharmacy in Primary Care (IMPPP) trial evaluated a complex, general practice-based intervention designed to reduce potentially inappropriate prescribing and improve medicines use for people living with polypharmacy. In this webinar, members of the IMPPP research team will share key findings from the trial, what worked and the challenges experienced, and how these lessons can inform future practice, service design and policy.
