Not USA, but we used to do this when I was a kid in the early 80s in middle-middle class, secular suburban Canada. Jennifer P, wearing a hockey helmet, rode her ten-speed off the jump and smashed face first-into the asphalt. I can still remember her little white front tooth embedded in the pavement.
