It's great that @shaunlintern.bsky.social has more stories to follow on this. As mentioned by others, there's lots of research going on about how to tackle #polypharmacy but one of the key challenges is what you define as a successful intervention. [1/2]

Almost 8 million over 50s are taking 5 medicines a day - the threshold for #polypharmacy - About 2.2 million take 10 drugs a day. For frail, elderly patients drugs can have cumulative, harmful effects and the NHS just isn't geared up to tackle the problem: #tomorrowspaperstoday

I've loved the Lost Words Blessing for so long and today I got to sing it in a beautiful choral arrangement with a fab scratch choir in Bristol. Very excited to hear the whole Spell Songs ensemble later this week too.

A vocal music score for the Lost Words Blessing. In the background, out of focus, are a group of singers and the conductor.

I really enjoyed a visit to the London Fashion and Textile Museum with my teenager this week to see the exhibition of work by Susan Collier and Sarah Campbell. It was as joyful as the name suggests: Paint! Pattern! Print! 💕🌈🌼🦚🦋 The striped jug in the collage below was his artwork in response.

A range of exhibits from Paint! Pattern! Print! showcasing the Textiles of Susan Collier and Sarah Campbell. The pictures show brightly coloured fabrics in mainly floral designs. There are also sketch books and a pencil drawing of a jug done by a visitor to the exhibition.

This does seem to be a key barrier when people ask how can we get the best possible people in parliament. Being a candidate and then a backbench MP looks pretty awful to me. "Why We Get the Wrong Politicians" by Isabel Hardman really opened my eyes to this and changed how I think about politics.

The visible, and sometimes openly expressed attitude of 'these people should be grateful. Last week they were a teacher and now they are an MP with a precarious majority and who gets death threats and called a paedo protector'. They liked being a teacher!

There are *so* many examples where the onus is on vulnerable patients to navigate poorly functioning systems. There are sometimes relatively simple fixes that could make things better if staff had time to pause and consider their service as a whole. But that's rare, and everyone loses as a result.

I know there's a lot of awful stuff on social media and that those who run the companies are interested in profit above any responsibility towards users, but I am trying to work out how to navigate this with my autistic 12yr old who gets so much joy researching his special interests on YouTube 😔

There is understandable concern with potential risks of data sharing in the NHS. There is also actual & real harm that happens regularly because there isn’t enough NHS data sharing. Changes to improve data sharing for NHS & social care services will make real positive difference to people’s lives.

Minister Zubir Ahmed, a transplant surgeon, told me that in January he had to decline a kidney for someone because a lack of access to GP records at a weekend meant he couldn't be sure the organ was safe. GPs meanwhile still rely on letters from hospitals to know what happened to their patient.

Really interesting session on #AdvancedPractice at CPC yesterday. Great to hear jointly from @rcpharm.bsky.social and @england.nhs.uk, and to hear some personal credentialling journeys. And thank you to Joseph Oakley for the follow up conversation, fascinating as always!

We’re doing the antidepressant thing again, it seems. I’ve been taking those two larger white pills since I was 16. No shame here. The little one stops my heart from freaking out. My brain catastrophically misinterprets threat causing an illness that has tried to kill me on at least five occasions.

In an open palm sit three white. tablets. Two oval lozenge shape and one little round one. There’s a ring on the wedding finger and some impressive lifelines across the palm.
