“Gender-critical” transphobia is fascism. Transphobes don’t want to “protect women & children”, they just hate trans people. There are no “gender-critical beliefs” just hatred. They are just a rhetorical cover for hate. They are certainly not in any way “worthy of respect” in a democratic society.

A far-right thug who performed Nazi salutes at a "protect women and children protest" has been exposed as a serial stalker and abuser.

I've been seeing references to some anti-trans changes coming to English schools in September. Now found out what it's about. Really not good. What it boils down to is making trans children's school time miserable, in the apparent hope it's just a phase and they'll stop all this trans stuff.

It’s non-binary people’s day. At a time where there’s so much anti-trans rhetoric, non-binary people are still targeted but are often left behind in the discussion & defence by others. Every trans-supporting cis person in the UK needs to make peace with becoming more annoying, persistent, and pedantic than they ever thought possible. EHRC guidance is full of contradictions - they're relying on transphobic compliance out of fear. DON'T GIVE IT TO THEM

Can we avoid #BreastCancer chemo when it *won't* give any advantage? Well, yes- for example, HER2+ can avoid the heart-destroying anthracyclines, and T1a + T1b triple negtumours don't need chemo at all. (Dr Andrew Redfern notes the triple neg and HER2 titles should be swapped). #RACS26

HER2 Positive
• All but very small T1 tumours need HER2 and chemoRx.
• For T1 disease taxane and trastuzumab very effective.
•In cases of toxicity 6 months of trastuzumab can be used especially if ER +ive or LN -ive.
• Can avoid anthracyclines, especially in ER -ive disease,
• Using NAC avoids T-DM1 for good prognosis patients.

Triple negative
• T1a and possibly T1b tumours need no chemoRx.
• For T1c disease taxane based chemoRx effective
• Could give neoadjuvant taxane with capecitabine for non-pCR.
• Capecitabine very effective in early TNBC. Can give adjuvantly.
• Can probably avoid carboplatin in post-menopausal women.
• The post-surgery component of
pembrolizumab may have no benefit. Trials should guide

Hormone Positive

• Grade 1. T1. LN -ive cases could avoid any treatment
• Adiuvant BabyTAM is promising after invasive ca. LoTam trial should confirm
• For G2. T2, NO consider genomic assays especially in young pts
• NAC and pCR give no guidance. Work on Ki67 drop is interesting
• Endocrine Rx even for 1-2 vears has big gains
• Little benefit of Al beyond 7-8 yrs. Consider in very high risk
