I've been at both ends of the stethoscope. Over-reliance on record leads to unhelpful assumptions. Not reading them leads to missing important stuff. Expecting patient to remember date & name of operation + pathology findings is unrealistic. I'm sure you do read notes. Surprised how many don't.

They don’t have access But you are the expert in you. What they need is a summary, the test results & you. Not everything. Too much reliance on a record leads to assumptions Things change & you change It’s vital as a clinician I start with you now, look at what we already know & then plan more

I'm looking for good recent article, by a human, in reputable journal about how docs (hospital or GP) approach patients in last year of life dying from causes other than cancer or dementia. Ideas? Names to look out for? I know how to use medline, but most of what I find is on cancer.

Does anyone even know what a neighbourhood is? A village? A town? Area within walking distance 9f a school/shop/ surgery? Population 1,500? Population 250,000? Word seens to mean different things in different parts of DOH/NHS-E speak. Not to mention ordinary human being speak.

Does anyone know what neighbourhood health is? There is no coherent rationale for it, no structure, no funding, no staffing, no premises. This ill thought-out plan is meant to be fleshed out through grassroots involvement, but there is little understanding & even less enthusiasm in general practice
