Online far leftists who’ve been fed a diluted form of Marxist-Leninism, most recently by the post-2016 DSA, mistakenly believe in powerful central committees in the shadows because that’s how communist “democratic centralism” works to set policy directives and provide orders to indoctrinated cadres.
That there is no Democratic establishment is closer to being true than the fantasy of an all-powerful monolith dictating the course of the party. There are various players and factions across the intra-party ideological spectrum but there's no united cabal issuing orders from The Democrats™ HQ.
The Democratic "establishment" is such a nebulous and disorganized thing they very often aren't even on the same page with each other. Consider how many contested primaries have senators, not just Sanders but other totally mainstream liberals, endorsing different opposing candidates.

The leftist idea that I've seen on here recently that democrats have NEVER helped anyone or done anything really devalues basic left-wing arguments. If the social programs of the past helped no one, why do more social programs? And frankly it devalues the millions of people those programs helped.
