As I often say, people behave weirdly at scale. We barely understand any of this! And just like a vaccine can end up failing clinical trials because it causes antibody-dependent enhancement, sometimes you try a thing and it makes stuff worse and you don't know why.
Remember Twitter's old "do you really want to post that?" toxicity detector? I remember hearing they did that on an experimental rule-out basis and didn't think it would have much effect, but it was wildly successful so they rolled it out to everyone with great bemusement.
So absolutely every safety intervention a site does is experimental! We're all just guessing at what might improve things, and sometimes the guess is wrong and it makes things worse. Or sometimes we try things to rule theories out and think it won't help and it does, massively.
I think two words will suffice: Pre-existing conditions.

I think about all the lakes and rivers and waterways in cities that used to be running jokes about toxic sludge and now are swimmable. Also we did kind of fix the whole ozone layer situation. Plus way fewer people smoke cigarettes!
